Faculty Resource Guide: Procedures for Obtaining Accommodations
Securing Classroom Accommodations - Incoming Students
- Students must complete a "Request for Accommodations" form, available from the SSD office or by downloading the form from http://www.susqu.edu/counseling/.
- Send the necessary documentation and the Request for Accommodations form to the Coordinator of Disability Services.
- Students will then receive a preliminary letter regarding your eligibility for accommodations.
- Students must schedule an appointment with the Coordinator of Disability Services in the first two weeks of classes. During that meeting you will discuss accommodation needs for each class and draft an accommodations agreement.
- Students must take a copy of the agreement to each professor. Discuss the needed accommodations. Both you and the professor should sign the contract. Return the signed agreement to Services for Students with Disabilities (SSD). The "Request for Accommodations" form will need to be completed each semester.
Securing Classroom Accommodations - Returning Students
- Students must schedule an appointment with the Coordinator of Disability Services in the first two weeks of classes each semester. During that meeting you will discuss accommodation needs for each class and draft an accommodations agreement.
- Students must then take a copy of the agreement to each professor. Discuss the needed accommodations. The Coordinator of Disability Services is available for consultation regarding suggestions or alternative accommodations. Both you and the professor should sign the contract. Return the signed agreement to Services for Students with Disabilities (SSD).
Arranging for Alternate Texts
- As soon as possible (June or early July for incoming students) students should contact SSD to discuss your specific needs for alternate texts.
- Students using Books of Tape from RFB&D need to register with RFBD individually. The cost for this registration will be paid by the University. Students. The SSD will work with the student to obtain the books.
- Inform SSD of your text requirements as soon as possible. We will:
- Request an electronic copy of the text from the publisher.
- If not available, the student must purchase a copy of the book and provide it to SSD. We will scan the book into a PDF file, or if necessary, a Word file, and place on a CD.
- The student is responsible for picking up the CD when it is ready.
- Assistive technology software is accessible at several locations on campus. Contact the SSD Office for an updated list of software and locations.
- Students who need assistance configuring their computers will be given assistance by the SSD and Information Technology Services.
Requesting a Course Substitution
If documentation supports and recommends a course substitution, SSD will make this recommendation to the Registrar and your Academic Advisor. The student will confer with their appropriate academic advisor and the Coordinator of Disability Services regarding specific courses that can serve as a substitution.
Securing Housing or Dining Accommodations
- Note that a single room assignment is only granted to students for whom no other accommodation is deemed reasonable, and only when available. A medical condition alone does not qualify a student for a single room assignment.
- Students must complete a "Request for Housing/Dining Accommodations" form. They will asked to describe the type of housing and/or dining accommodation requested and how your disability impacts your daily life. Submit the form to SSD.
- A physician must complete a "Medical Information Request form." The form should be sent to the Coordinator of Disability Services who will review the form.
- If approved, the student will complete a Housing Accommodation form to submit it to the Residence Life Office.
- Residence life discuss the request with the student and work to accommodate the student's needs. Residence Life will notify the student in writing or by telephone about the house arrangement.
- If dining accommodations are requested, SSD will consult with Dining Services regarding possible accommodations.
